Claim Request
To file a claim, complete the Claim Form and submit along original paid medical bills. We recommend you to retain the photo-copies of all papers
(claim form and bills) for your record and send originals by "Registered certified mail" to ensure the confirmed delivery. Remember, you are
responsible for your deductible, coinsurance amounts and non-eligible expenses. Please remember to submit your bills and receipts as soon as you
receive them. Do not hold them until the end of the year.

Cancellation Request
Please, send request by email or fax to Insurance company along with given information below:
  • Certificate number.
  • Name of insured person.
  • Date of birth of insured person.
  • Requested date of cancellation (can not be today or previous date)
  • Reason of cancellation.

Cancellation will be effected from requested date or date on which the request is received by the Insurance company, which ever is later.

Important note: If you have submitted any claim and after that you want to cancel the plan, then the premium would not be refunded by any Insurance
company. For
example - Suppose you purchase the plan for 5 months. In the first month, you had an incident for which you had to visit the doctor and
doctor has send the bill to company for claim. If you want to cancel the coverage from the beginning of the second month, you would not get any refund
from the company because you have already used the coverage. This is Insurance company policy and applies to all the plans

Effective date - The plan start date.
Pro-rated period - Suppose you purchase the plan for 5 months and after 3 month and 20 days you want to cancel the plan. The company would refund you premium for 2
months and 10 days.
Fully unused months - Suppose you purchase the plan for 5 months and after 3 months and 20 days you want to cancel the plan. The company would refund you premium for 2
months.
Used month - Suppose you purchase the plan for 5 months and after 3 months and 20 days you want to cancel the plan. These 20 days are part of forth month which would be
considered as partial month.
